Upon the approval of your online application, you will need to submit submission the following documents in order for your streamyx service to be activated. Please fax all relevant documents to 03 2096 9190.

For Individual Applicants:

For Limited Company (Sdn Bhd):

For Sole Proprietor or Partnership:

For Foreign Incorporated/Representative Companies:

b. Photocopy of Ministry of International Trade & Industry / MITI letter (Foreign Representative Office)
c. Signed and stamped application form or confirmation letter from TM Net

 

Important Terms & Conditions:

  1. tmnet streamyx basic package is applicable for personal application only.
  2. Minimum subscription period is 12 months for all packages except with modem rental and other promotion packages.
  3. Service is subject to availability and technical testing.
  4. Speed of service is on best effort basis.
  5. Customer is NOT ALLOWED to change the telephone number (connecting to tmnet streamyx) before installation of service.
  6. Customer is responsible for internal wiring, NIC card and networking.
  7. Self-installation customer is required to prepare own modem before the 7-day auto account activation.
  8. For tmnet streamyx in-a-box (SIB) & without modem package: Auto account activation is 7-days from the date of registration for self-collection and 10-days for courier.
  9. RM88 installation fee is applicable only if installation assistance is required.
  10. For account activation or technical support. please call 1-300-88-9515
  11. Streamyx in-a-box (SIB) courier service charges are RM14 for Peninsular Malaysia and RM25 for Sabah/Sarawak.
  12. First bill received will include the following fees: RM75 for activation + RM88 for installation (if applicable) + First and Second month advanced subscription fee.
  13. If customer terminates tmnet streamyx before the end of the minimum subscription period, the customer must pay subscription fees of the remaining months and return the modem. Pre-mature termination includes relocation of premise.
  14. tmnet streamyx bill must be paid before the due date to avoid any service suspension. During suspension period, TM Net will continue to charge for the monthly subscription fee. RM10 reconnection fee will be charged upon service reactivation
